• ✗Я хочу записать к врачу.
  ✓Я хочу записаться к врачу.

  Use the reflexive verb записаться (to sign oneself up), not записать (to write/record something).

• ✗У меня болит горло три дня.
  ✓У меня болит горло уже три дня.

  To say how long something has been happening, add уже (already/for).

• ✗Я иду в врачу.
  ✓Я иду к врачу.

  Use the preposition к (to, towards a person) with the dative case, not в.

• ✗Мне нужно сдать анализы.
  ✓Вам нужно сдать анализы.

  The doctor speaks to you formally using вам (you, formal/dative).

• ✗Я пью лекарство.
  ✓Я принимаю лекарство.

  For medication, use принимать (to take), not пить (to drink), which sounds unnatural in medical contexts.
