자주 하는 실수

• ✗保険証があったら、3割負担です。
  ✓保険証があれば、3割負担です。

  For general facts/rules, ば is more appropriate than たら. たら is for specific one-time events.

• ✗大きい病院に行くならば、紹介状がいる。
  ✓大きい病院に行くなら、紹介状が必要です。

  なら does not combine with ば. Also use 必要です for polite speech.

• ✗全員が保険に入ることにしています。
  ✓全員が保険に入ることになっています。

  ことにしている means a personal decision. ことになっている means an established rule.

• ✗手続きは必要があります。
  ✓手続きが必要です。

  必要です is the correct pattern. 必要がある means there is a need (different nuance).

• ✗保険証を持ちですか。
  ✓保険証はお持ちですか。

  The honorific form is お持ちですか. Also use は (not を) for topic marking in questions.
