Asking about dosage:

Checking safety:

这个药有什么副作用吗？这个药有副作用吗？副作用 already implies a specific meaning — adding 什么 makes it awkward. Use the simpler form.
我头不舒服。我头疼。头 alone does not express discomfort — 头疼 (headache) is the correct compound word for head pain.
药剂师说我一天要吃三次。药剂师说我一天要吃三次药。The object 药 is needed — without it the sentence is incomplete: eat what three times a day?
我对这个药过敏的。我对这个药过敏。No 的 is needed at the end of a simple factual statement — 过敏 alone closes the predicate.
