Health Insurance and the System in Japanese
Understanding the Japanese healthcare system requires conditional forms:
〜ば (conditional: if)
The ば-form expresses a general condition.
- 保険証(ほけんしょう)があれば、3割(わり)負担(ふたん)です。 (If you have an insurance card, it's a 30% co-pay.)
- 予約(よやく)すれば、待(ま)たなくていいです。 (If you make a reservation, you don't have to wait.)
〜なら (if it is the case that)
Used for situational conditions.
- 大(おお)きい病院(びょういん)に行(い)くなら、紹介状(しょうかいじょう)が必要(ひつよう)です。 (If you're going to a large hospital, you need a referral.)
〜ことになっている (it is the rule that)
Describes established rules or regulations.
- 日本(にほん)では全員(ぜんいん)が健康保険(けんこうほけん)に入(はい)ることになっています。 (In Japan, everyone is required to have health insurance.)

常见错误
保険証があったら、3割負担です。保険証があれば、3割負担です。For general facts/rules, ば is more appropriate than たら. たら is for specific one-time events.
大きい病院に行くならば、紹介状がいる。大きい病院に行くなら、紹介状が必要です。なら does not combine with ば. Also use 必要です for polite speech.
全員が保険に入ることにしています。全員が保険に入ることになっています。ことにしている means a personal decision. ことになっている means an established rule.
手続きは必要があります。手続きが必要です。必要です is the correct pattern. 必要がある means there is a need (different nuance).
保険証を持ちですか。保険証はお持ちですか。The honorific form is お持ちですか. Also use は (not を) for topic marking in questions.
